Yeah, the audio in this game is hit or miss it seems in regard to locating enemies.
Sometimes it's decent and I can ascertain what direction the enemy is approaching from and other times they are either silent or the footfalls sound like they're coming from the complete opposite direction.
I've messed with my audio setting in-game and on my PC and even went and upgraded to a THX Surround Sound set of headphones and the problems still persist.

Unfortunately the audio of Frostbite hasn't improved since BFV/BF1, which had the same problem of sounds sometimes working and sometimes not.
Sounds behind / above are some of the worst problems and getting shot in the back by silent infantry or vehicles happens way too often.
I've tried every possible in-game setting, tried stereo and different 3D audio systems (DTS, Atmos, Windows Sonic and Boom3D) and the problems are the same, some sound effects are just not played at all, no matter what setting or 'audio enhancement' I use. Also tried using both the motherboard audio and a hifi-dac, again no difference (except for the quality of the sound).
Dice needs to make BF-games fun and things like the "bad" audio can be really frustrating.
