Same issue here.

Also when you throw a grenade, wait 30 seconds for it to replenish, and then throw again it throws a smart grenade instead, even though you havent changed the grenade type. 

Same issue for PC also.

Grenade doesn't show correctly, and it seems random.

Looks like frag grenade, but turn out to be vehicle grenade, or vice versa, and sometimes its correct, really destroys my game play.

This is still broken for me. Switching grenades simply doesn't work most of the time.

Now the problem is that if you switch from any weapon to the grenades the grenades also switch. I want to throw a vehicle grenade and then when I throw it turns out to be a cluster grenade. Can they please fix the sensitivity of this, it has got me killed so many times.
