Unable to respawn or be revived after being downed

I have a fix and a kinda explination

You're body still takes damage and the game doesn't think you're dead.
If you get a teammate to lay down in your body enemy fire can damage you and kill you allowing you to respawn again. 
This appears to be a state-changing bug where your character is no longer in your control but the game doesn't actually consider you to have lost enough health to be dead.
