I have done everything you have listed above and the achievement didn't unlock for me. I even went back to the multiplayer many times doing the assignments over and over I thought the achievement might have been delayed but I'm pretty sure it's glitched at this point. If you want I can provide a picture to show proof of what I mean.
I believe you. The only thing that I can recommend is to contact an EA advisor. The EA advisor should be able to help you.

- @danisoff How long does an EA advisor usually get back to someone? I'd hate to push this any further if it's not an issue they deal with.
If you use the chat option it would take maybe 2 minutes, if you use the email option it could take days.
