Battlefield 4 missing campaign collectible missing

So I've gone through the mission where you go to the prison (not sure the name of that mission off the top of my head) and I've got all 3 dog tags throughout the mission and have even looked up to make sure that's the location in case for some reason I was missing something. At the menu at the end of the mission it tells me I only got 2/3 dogtags.. So what's going on there? Because I went through on each difficulty grabbing all 3 and still nothing.. I believe it's doing this with one other mission as well. Thanks!
