You are having some weird problems and I don't think a patch will fix anything as no one else has this exact problem and thus it's probably something going wrong on your end.
It's nice to see you have put some effort into writing about your problem 🙂 I however would like to know the computer specifications of the computer in question. If you can upload your dxdiag information to hastebin.com that would be great as well.

A screenshot or video of the problem occurring if that is possible is also very helpful.
From the information you have given the first two suggestions for solutions I have are:
Check Hard Drive For Errors And Corrupted Files
All files can become damaged. This is just the nature of hard drives. If this affects Origin or it's games this can cause errors and it's therefore a good idea to scan your hard drive. Do this for all hard drives that has Windows, Origin and your games installed. Please tell if it finds something.
- Go to "This PC" in Windows and right click the hard drive and click "properties"
- Go to "Tools" and and press "Check"
- Click "Scan Drive" and let it scan for errors.
- If it finds errors let the program fix them
(If there are errors it could mean nothing, but it can also suggest that it might be time to format your computer and/or replace the hard drive) - Reboot your computer
- Run the scan again
- Keep repeating step 3-5 until no errors are found.
Clean reinstall of graphics driver
There's plenty of guides on how to do this on the Internet, but basicly you download the latest driver, then you uninstall your current driver and then you find all files and registry entries that it has left behind and manually deletes those as well, then you reboot the computer and install the new driver.

http://imgur.com/a/XIgY0 Here are a few screenshots I was able to get that illustrate some of the issues. There is a red tint over things, but I do not know how this map is supposed to look. There were no FPS/lag issues. I just looked this map up and the playground in the center of the apartments is not visible at all.

1) Both drives were scanned and there were no errors found on either.
2) There seem to be remnants of an old AMD card that are still in my system. Could this be causing the problem? It was uninstalled via the remove programs feature that comes with windows and does not show in that list anymore, but DDU was able to see it. I checked task manager and there were two AMD processes running still. I have NOT removed or reinstalled the driver yet, but I am prepared to do so in a few hours.

So, your hardware is good enough which is good to know 🙂
From your dxdiag I can see your DllHost.exe has crashed. This is a Windows file and it should not crash, so just to be sure lets do a quick check on your Windows installation:
Check Windows for Errors
To scan your Windows installation for errors open a command prompt with admin rights and run the command: "sfc /scannow". A more detailed guide on how to do this can be found here:

Please tell if it found anything.
Now, especially the last screenshot you linked looks like classic artifacting, this can be caused by a lot of things. The most common is that your graphics card runs too hot. Have you tried measuring it's temperatures while the problem occours? It could also be the first signs of a dying graphics card but lets hope not.
As from the AMD services still running your motherboard and CPU is still AMD based so be careful 🙂

The graphics card is only a month or so old, so I certainly hope it's not dying. That artifact does not happen on all maps, and has only appeared on two that I've tried so far. Loading up another game just now, it was not present and the sky appeared normal, but the buildings were still mostly invisible.
I do not know how to check my GPU temp, but the CPU is running in normal range and I do not have issues running other games on my PC.
I left the AMD processes alone and have not touched the drivers yet, so no worries about being careful there. 😕mileytongue:The scan just completed, it stated "Windows Resource Protection found corrupt files but was unable to fix some of them. Details are included in the CBS log." I found the log and at the end, it states: 2015-05-26 11:02:25, Info CSI 00000907 [SR] Verify and Repair Transaction completed. All files and registry keys listed in this transaction have been successfully repaired

http://www.techpowerup.com/gpuz/ is a good universal tool for checking temperatures.
As for the Windows Error Checking tool that message means it found more than one "broken" windows files and were only able to fix some of them. You should keep restarting and running the tool again until it has fixed them alll and comes up with no errors. You might need to boot into safemode to fix it all:
