Whenever I play no one can hear me using VOIP. Also, I hear other people's VOIP through my speakers even though every other bf4 sound comes in through my headset. I have the headset set as default communication device and default device.

In order to talk through VOIP in-game, you have to hold left Alt button. Are you sure you are holding that specific button when trying to talk?
If you are still experiencing the same problem, let me know if you use USB or 3.5mm audio/microphone jack headset.
