Bug with automatic knife action when close to an enemy on Xbox 360

There is a really annoying bug on Xbox 360 that when you are near an opponent, pressing the trigger performs a knife swipe versus gun fire. I have switched controllers 3 times and it happens with every controller. This happens with all player classes, typically when some sort of long gun is being used, and sporadically when a pistol is being used. You are too far away from enemy to do any damage, and if you are close, they are able to do a knife reversal on you and kill you.
