Your whole story and experiences you have been going through are exactly the same as I have/am!!!
Except that your server is Shanghai 24/7 while mine is Metro 24/7.
And your platform is PC while mine is PlayStation.
(think we are both in Europe, so our servers are in the same data center as far as I know)
Your summary of 'the 3 typical server crash scenarios' are familiar, though I can solidly confirm that type1 is not possible on the PlayStation platform. And the DDOS in its traditional form would, as you write, typically be able to be handled swiftly by the server-hosting-partner that you contract with for PC and that EA contract with for aka me on the PlayStation platform. I have conducted a substantial and detailed tech monitoring collaboration together with the BF4-server-hosting partner in Europe (they even shared some of their tech tools w me) and we could both agreed to our shared findings that there was absolutely nothing wrong with the lines between the server and the connected BF4 players. But the problem was happening in EA's connected backend, referred to as BLAZE. And these were the concluded outcomes from a methodologic and technical detailed analysis and monitoring over a 7days 24/24h period. So despite a perfect running server and a perfect connected player client, with zero internet line drops and zero package loss over a 7 days period 24/24h between the two of them, then the EA BLAZE backend kept dropping the players off the game server again and again.
And I am, as you are, troubled/harassed by the third type of hacking problems (The "Blaze Disconnect") and having exact same experiences also as you describe so well in your history so far with your very popular BF4 server:

We are "The Next Era", a well-established community centered around our 24/7 Siege of Shanghai BF4 Server. Ever since we founded the community in 2018, we have been attacked from many different people, usually because they did not agree with our rules, wanted to force players to join alternative servers. We even got offers to pay ransom money to not be attacked. Since a week, our server is again under attack, being crashed up to four times - per hour. This will lead to the slow death of our community, and one of the last Shanghai servers with proper moderation.

First I was threatened that my BF4 server would be crashed/attacked if I did not unban certain named players.
Then I was offered cash to sell my BF4 server (by these same individuals).
Then I started receiving direct threats about my server being brought down.
Then specific warnings ahead of time and later with specific time points when to expect the server crashes. And then the server started crashing at these specific time points. And since then, its just crash after crash after crash after crash....
Multiple tickets raised with EA/DICE.
3+ weeks in and still no resolution.
