Happened to me in BF3 too. But only recently...when I played BF3 a long time ago with friends it worked fine and I had no errors.
With BF4 it let me play 3-4 games and then I stopped playing. When I tried to play again this happened. Sometimes this error comes up, and sometimes BF4 just crashes. It's a wierd crash though because my mouse laggs really hard...Never seen that before.

in-game, I've set my refresh rate to 50.
so far, that seems to be working for me.
My monitor is the Asus ve248

Would be really nice for DICE or EA to at least respond about this issue..

For what its worth, I had the same exact error with BF3. I NEVER had any problems with my pc running BF3, beta or retail game. Then one day about 6 months ago I started getting the DirectX function GetDeviceRemovedReason failed with dxgi_error_device_hung error. Didnt change ANYTHING in my system, hardware or software. Long story short, it was a HEAT ISSUE. I had/have, at the time of my issue, a HD7970ghz card. Never gave me an issue with BF3 or any other game. I ended up removing the heat sink and fan. I noticed on the gpu, there were areas that NEVER had any thermal paste on them. I could see my reflection from the GPU, not good.. I ended up cleaning off the bull**bleep** TIM (thermal interface material) they put on at the factory and used Artic Silver 5. Put everything back together and never saw that error again. All I did was reapply the TIM and problem solved. Granted I used AC5 but you get my point.
Now this might not help everyone or anyone. But I figured I'd share my "fix" to this issue on my pc. Hope you guys can figure out what is going on with your rigs. My money is on the GPU's overheating. Poorly applied TIM can happen to Nvidia and AMD.. just food for thought.

The problem is about 90% nvidia and the rest ATI.
and so far, I've seen twice that people with ATI cards that are overclocked (and thus overheating) have this issue.
The rest of us are nvidia and my issue at least, is not heat related. My GPU runs around 161ºF
