Fourth Floor Dog Tag?

I've looked everywhere online to see how to get it but nobody has cleared it up as yet, me and my father both play xbox one battlefield 4 a lot. We have both played BF Hardline Beta and both have the phantom assignments all done. But none of us have the Fourth floor dog tag.

    As I recall that dogtag was a mess. Some people got it that shouldn't have.

    The intended requirements I believe was logging in on some certain days, having the beta and visiting battlefield.com/4thfloor but that might be wrong.
