[INFO REQUEST] PS3/XBOX360 freezing/crash issues

We need some of your information in order to investigate some freezing issues with the PS3 and XBox360 console. It would be great if you guys could provide us with the following data, it will help us...

What i did was I logged onto another account and works and i tried everything, but bad thing its not my account its my gf's and its on my PS3 so its not an issue w/ the PS3 its something w/ the account. I played it first on my account and downloaded the 1.02 patch and thats where the problems started for me but my gf started to play yesterday and no freezes. I also tried that field upgrade and nothing. Please fix it, BF is my crack im dying here.
