[INFO REQUEST] PS3/XBOX360 freezing/crash issues

We need some of your information in order to investigate some freezing issues with the PS3 and XBox360 console. It would be great if you guys could provide us with the following data, it will help us...

I did as suggested by eabastian at start of this post and my xbox still freezes on random maps during load screen.  I know I have locked up because the white curser at upper right side of screen stops moving during load.  So you need to remove solved because this is anything but solved and it is misleading.  In fact is worse now that I did as suggested.
