1. Attempting to install the game on ps3 HDD
2. at the first blinking cursor at the top right, just after "syncing trophies"

5. my opinion is someone dropped the ball on the disc printing, and the disc it self it faulted.
got an error "To play this game you must insert the following disc. Insert the disc, and then start the game again. (8001003D) Battlefield 4"
so I deleted game save data and cookies, history, cache, reinstalled then download update, game restarted and froze on blackscreen.
Restarted PS3 got into an online match and...
1. at the end of match, showed a blurry background then blinking cursor, game froze but not the PS3, was able to quit thru XMB.

for troubleshooting purposes, I have tried BF3 and GTA and other BDROM based discs, they all perform without any issues.
Attempted to try again and my PS3 rejected the Battlefield 4 disc, it would not recognize it. This was followed up by a quick game of GTA5 and now playing BF3 with no issues.
Also 2 of the 20 times it did not freeze the PS3 I was able to play "test range" the first one had no sounds and no textures and very low poly models, the second time had no sound except a few sounds that were no where the quality I am used to with DICE games and a few models that were low poly.
Photographic and video evidence can be provided for (8001003D) code and "no sound test range" upon request.

I have the same issue and don't have a disk got mine from digital downloading it. They have a software issue hope they fix it soon or this could be the most embarrassing game drop ever.
