[INFO REQUEST] PS3/XBOX360 freezing/crash issues

We need some of your information in order to investigate some freezing issues with the PS3 and XBox360 console. It would be great if you guys could provide us with the following data, it will help us...

This issue is currently not fixed . Today is 11/2/13 at 2:14 pm pst, and I just got off the phone with EA and he told me to post this. Any time I join a game, I take two steps and it freezes, forcing me to do a hard shutdown on my ps3. Not cool! I also experienced on campaign mode when I first tried it, it would not regenerate my health. Please find solutions to these problems fast. Hasn't EA been making battlefield for like 20 years now? Why so many problems?
