According to these patch notes, they fixed the crash that occurs when the map changes after a match. But, I downloaded the patch, that crash still happens with me, EVERY SINGLE TIME! Without exception! It is maddening. And yes, I have updated all my drivers, even downloaded the beta drivers, I have cleaned my PC, attempted to repair my game install, re-downloaded the game, turned off my virus software, tested my memory, etc. If there is a "fix" posted somewhere on the internet, I have tried it. The problem lies with the game.

To those that are experiencing the crashes have you tried what I posted on this thread ? (Message 291) I was having the same disconnections without any errors to the desktop; I posted a link which presumably fixes this issue, after running what mrnighttemple (credits go to him) posted I do not get disconnected anymore; its worth giving it a try.
