The first pic. you posted does exactly tell you what to do! It's kinda difficult to do but it's possible. So, make sure you chevk this War Story Mission again and make sure you do this required assignment for the Codex.
The others you shared are simply to explain.
These are not from Single Player!
It's Operation Campaigns from the Multiplayer what they mean there. 🙂
What are Operation Campaigns?
Well, in the BF1 Menu head over to Multiplayer > "Operations". There on the screen you can see on the bottom right corner when a Operation Campaign is live.(A Operation Campaign is always a combination of two Operations!)
Different Operation Campaigns go live randomly, always for like 12 days normally. If you completed a Campaign once, it'll give you the Codex.

@Its_Travis_199 you obviously didn't read my first comment..... I said " I have completed them BUT IT DIDNT GIVE ME THE CODEX" I have all my trophies shooting down 10 planes within 30 sec is a trophy requirement so if I have the trophy and the requirement then why hasn't it given me the codex
@Its_Travis_199 Also when I click on the war stories it shows I have all the requirements for each mission so again why hasn't it given it to me

You asked for the Campaign (Single player Codex Entries) and as you posted the screenshots later on, i could see it's actually not been Single player Campaign Codex Entries.
Just the first one with the Planes! 😉
So that's why i said you need to play those Multiplayer Operation Campaigns to get them.
The Plane Codex was might just Bugged, you could try again if it unlocks then.
@PSYCHO-TERMINATR under Codex Entries "The War" from the 105 possible, not all of them are from Single Player!
