Once you have purchased premium, all of the camo's and tags and everything should be available. Be aware though that some of them will only come from Battlepacks, as that is how they were granted to everyone else.
Check the Battlepacks tab on the in game menu to see if you have any Premium packs to open, open these and you should unlock everything that is supposed to unlock.

I checked in the battlepacks tab and i only have gold and the other battlepacks earned through leveling and weapon progression, I also tried clicking on the phantom skull on the leaderboards page and when the terminal-like screen came up it showed my name and said PREMIUM not detected.
Have you tried re-installing Premium?
Not sure because I bought It as dlc and I'm not sure how to uninstall dlc on ps4 without uninstalling the full game.
Sorry for the delay in getting back to you. From what you described about the skull and premium not detected, it might actually mean you do not have premium. If you purchased each DLC separately then you will not gain access to the premium features. Additionally, if you are sharing premium (by this I mean another account purchased it and is sharing it with yours) then you also will not be entitled to the full premium features.
If either of the above are true, then I am sorry to say but you will not be able to get the camos tags or be able to complete the Phantom assignments.

Firstly, I was able to download all dlc for free, secondly the attached file should show that I have bought premium.
Do not forget just because its downloaded doesn't necessarily mean its been installed. Go to BF4 game menu "STORE" and install from that end (premium and DLCs) Premium 1st, it won't charge you it should show free. Also look at your library that's another way to make sure they have actually been installed.
