I see your driver is pretty new.  This is usually a good thing, but the recent drivers have been causing alot of FPS problems for Battlefield 4, especially on older cards like yours.  My recommendation is to roll back to either the driver released on 3/1/16 (362.00) or 3/28/16 (364.72).  You can find old drivers here.  When you do it, make sure to do a full clean install of the driver or it may not work correctly.  If you do not know how to do this (removing the old driver fully, etc), let me know and I will post a full step-by-step process for you.

EDIT (to help users who view this thread in the future):

Here are the steps to do a FULL CLEAN install of your graphics drivers:

1.  Go here and find the version of the driver you need, and download it

2.  Save it to a location that you will easily be able to find (such as your desktop)

3.  Download and install DDU Uninstaller

4.  Run DDU Uninstaller and remove all drivers present (there may only be your current one, but sometimes an old driver will still be hanging around that you did not know was there)

5.  Reboot your PC (when it boots back up, your screen may flicker and your resolution will probably not be correct, this is normal and will go back to normal when the driver is installed)

6.  Find the downloaded Driver you saved before and install it

7.  After the install is complete, Reboot the PC again

8.  After your PC boots back up, everything should be back to normal (if your resolution does not automatically reset itself to your monitor's native resolution, then go to your settings and set it to the correct resolution)

9.  Boot up and test the game again

    thanxx for your help............plz show tell me how to do a full clean install of the driver. and another thing whenever i am running bf4  my cpu usage is at 100% constant. i have played other games like gta 5 and witcher 3  without anyprob, facing these probs only in bf4

    dont you think for 100% cpu usage i am getting low fps??

    tried clean boot but its same.i am  not having this 100% usage problem while running bf4 alone plenty of others are there....but no solution can you guys help plz

    Glad to see you got it working.  I edited my post to include the steps for a full clean install of your graphics driver, more-so for users that check this thread in the future.

    sorry have to add this, have you heard or an application called Razer Cortex?

    it optimizes game, will increase fps at the same time disabling applications not necessary to gaming.

    just google search [ Razer Cortex]

    if your net connection is slow, there are programs that will also optimize your connection as well.

    I'm sorry, but I'm gonna have to object to that suggestion.

    1.  The OP's issue has been solved via rolling back to a older driver due to the buggy new drivers, it was not an issue of optimization

    2.  Programs like Razer Cortex and others (such as Nvidia GeForce Experience), do not optimize games correctly.  Back on my last GPU, the GTX 760, it was telling me that my "optimal" settings were Ultra, which is far from what they were.  Yes the 760 could handle Ultra settings, but barely.  Had low FPS and fluctuated very frequently.  Those programs do not optimize correctly.

    The OP's issue has been solved, and they no longer need to do anything.
