I had the exact same problem. Took me like 20 times until I tried something stupid enough that it worked. It's not a technical problem like xxxo77o4906 indicated, it's just something stupid that actually went through Q&A before the game was released.
Basicly you run, either jump or don't (can't remember) and then you let go of your mouse and keyboard - DONT TOUCH ANYTHING, empty your hands and raise your fists in the air in anger of silly gameplay decisions until he figures out that grabbing the ladder might be a good idea.
