Did you use the code on Xbox One and later delete the file or did it stop working after a title update?  The china Rising entitlement file (not just the expansion) must be physically present on your Xbox One's HDD in order to access the expansion, and sometimes it's corrupted during title updates.  The solution is normally to go into your Xbox One's purchase history and redownload the China Rising entitlement file, completely close BF4 and restart the game so it initializes with the entitlement file present granting you access to the expansion.
