Same happened today, twisted steel, frontlines. I apparently spawned with the bomb each time I died. I'm not sure if others could see that. I planted A and B, but the last, C, were so strongly defended I could not get through. I think the same happened to the other bomb too, because there were no new bomb respawns.
Happend to me also 4-5 times already. On PS4 in grand operations. Hope that they fix this asap.
