You need 17 Super Gametime Tokens (170 regular Gametime tokens) for a 93 OVR Sam Wesloski (SAM).

Yes same here. Also don't know what the point of getting another token is or what reward you inevitably get. I hate the inception style sets.

This just happened to me when I exchanged mine, definitely a bug. I received no super token.
