If you bought the game from a brick and mortar store new then you should return the game to them and request a replacement. That will be your fastest and easiest recourse.
If you purchased the game from outside the country, the codes are region locked. This means that a European copy may not work in the United States, etc. This is primarily done by publishers because some countries have unfavorable exchange rates that cause out-of-country sales to be less profitable.
If you have an Xbox that you originally had in one country and you moved to another, this can also cause a similar problem. In that situation, you will need to purchase a copy that is region locked for the country you were in when you set up your online account or you will need to create a new account for your current country.
