I'm on a ps4 and about 10 hours into the game my audio started to crack/hiccup while in game. I go back to the main game screen or ps4 launch screen and the audio is fine, but when I load my character the audio cracks again. This sound issue makes the game unplayable.
I don't know if you can do this on the PS4 (Or PS3) but try changing your audio options from whatever it is to stereo, or if you are on stereo, switch it to another option and see if that helps.
It did for me.
Today the sound went and I fixed it by going into options and changing the Dynamic Range Mode from TV to Home Theater and the sound was back!
