I think issues like this can happen when the game assets aren't loaded into memory quickly enough. I had isses like the Maryden glitches you described, as well as some characters in Skyhold being invisible. It got a bit better when I installed the game on an SSD, and the issues are now completely gone since I upgraded to a high end GPU (Radeon R9 390 8GB).
I know that's not very helpful, but I figured I may as well share my personal experiences.

I know that's not very helpful, but I figured I may as well share my personal experiences.My pc is fairly high end so I doubt that's the issue, and remember I never had the problem back in Haven...
