XBOX one player, I had the same issue, no Chest in Val Royeaux prison during Blackwalls personal quest. i went back to previous saves and tried a few times to get it to spawn but never could. I have seen youtube videos of where the chest is and that it did spawn in others games. No idea if its patch related or based on story/character choices i had made.
