I think his approval needs to be high. I completed four playthroughs and his personal quest only triggered in two.

I think it's a lost cause at this point. I exiled the Grey Wardens and did some other stuff he didn't approve of. It's hard to recover from that.

You could go to the Storm Coast or the Western Approach to kill darkspawn with Blackwall in the party. His approval increases a little when you kill darkspawn.
