[Bug] Broken ladder in Din'an Hanin - Upper crypts - can't get back up

Not sure how to precisely describe it's location.  It's next to a blocked stairway that would lead back up, but even bunnyhopping I can't use it.

The ladder is broken.  When you get within range, it highlights and shows "climb" tooltip, but you have the standard pointer, not a hand, and clicking does nothing.  If you move the mouse to the left, into the wall, the ladder is no longer highlighted, but an "interaction hand" appears - but since your "hand" is not over the ladder, clicking still does nothing.

From a distance only "climb down" at the top is in the right spot.

There is also a quest item in Emprise du Lion in the little blue house with the woman lamenting a ring that seems to have the same issue.  Box on table appears with interaction outline, the spot has a ! on the map as a new quest, but you just can't interact with it.  

The interaction "zone" for these objects seems offset from the object in the world, preventing them from working.

Edit: workaround!  You can indeeed, with some perserverence, use the stone blocks at the top of those stairs to get up and around.

Side note: once i got back to the top, the companions didn't have any issues and used the ladder to follow me just fine.
