Capturing Caer Bronach Bug

I'm playing Dragon Age Inquisition on the PC. I have all the current DLCs installed as well as mods. I'm playing through after Patch 12.

I'm running into an old bug, and many of the suggested fixes aren't resolving the problem.

I get to Caer Bronach in Crestwood, fight my way through it, and finally go to raise the flag. The cutscene of the flag rising while my party and I watch plays, then I see the loading screen with lore cards for about two seconds. After that, the screen goes black with the flashing DAI logo in the corner. Eventually, it stops loading and completely freezes. The only way to get out of it is to end the task via the task manager. 

I have attempted the following fixes: 

1. Removing all mods

2. Lowering graphics 

3. Changing the party (Viv, Dorian, Cole & Dorian, Cole, Bull & Dorian, Cole, Cass & Sera, Dorian, Viv & Viv, Solas, Varric)

4. Loading an earlier save

5. Going halfway through the quest, fast traveling out, and going back in on foot

6. Repairing the game through Origin

7. Restarting both Origin and Steam

Nothing has worked. I need help!!!
