It usually happens to me whenever I use one of the horns that allow you to change your party around. Good example would be travelling to the Hinterlands with Dorian, Iron Bull and Cole who all chatted about different things. After I change the party to Varric, Cole and Solas, who I'm 100% sure have a lot of dialogue given they all have a mission together, they didn't say a word for 3 hours of running around the Hinterlands. Even when destroying Red Crystals for Varric, there was no response for it.
So I suspect it might be an issue with using the change party horn!
