I'm having this problem as well. I'm over 40 hours in. I've nearly cleared the Hinterlands and Storm Coast, and have visited a few of the other areas. I've had less than 10 banters for sure, three of which triggered in a row walking down one hill in Storm Coast.
I'm on PC, Digital Deluxe download edition.
