This is a bug that seems to affect many NPCs. In my game, Varric was missing from Haven at the start (was not by the campfire) but his icon was there. Once I went to the Hinterlands and traveled back he appeared. Can't say for certain it is the same bug though. Right now I'm early game and Cullen is not at his icon (out front of the Chantry/War Table), though I have traveled back and forth to different maps a few times.
