If you are using a Windows utility called MKLink to redirect DAI to look for your save files on another drive, DAI seems to ignore the MKLink redirect. So upon finding no ProfileOptions file, it resets everything to default.
Unfortunately some programs ignore MKLink and DAI is one of them. You will have to leave your files where DAI expects to find them.
Thank you very much for your reply. Sadly I don't think this covers my issue. I'm not using any software like that, and all of my Origin games, saves, etc etc, are completely default.
I've tested it a bit today, so far it only seems to occur when I reboot, which isn't often, this rig stays on 24/7, but lately I've needed to for updates/drivers.

Something must be up with the game install then, because all the graphic settings and keybindings are stored in My Documents>Bioware>Dragon Age inquisition>Save in 2 files caled ProfileOptions and ProfileOptions_Options (I believe, going off the top of my head).
If those files are being deleted or somehow can't be found by the game, then I would expect your issues.
I would suggest then try running a repair on the game (in Origin, right click on DAI in "My Games" and choose repair). Perhaps the Windows updates messed with the registry, and DAI just needs to reset something.

Just a shot in the dark but it sounds like it could be a write permissions error. Have you tried launching the game as an administrator?

Testing now, may take some time for me to be certain either way. Gonna try forcing it to run as admin.
