Having the same problem. I've tried reloading multiple times including from earlier saved games as far back as the snowy sequence before arriving at skyhold. Officially stuck unless someone else has found a workaround for this?

Still the same bug. It's been 2 months since I played the game and it looks like it will be much longer at this rate.
It is not a bug, it is a design choice. The CC differs immensely from DA2 hence impossible to re-create the same appearance from DA2. People in this forum need to differentiate between a design choice and a bug.
The design choice can be horrible but it doesn't make it a bug just because it does not please you. And you always have the option to edit the way your Hawke looks, it is not like you are forced to accept a pre-rendered Bioware pill-sized Hawke.
I don't understand. When the choice is to "Use the iconic default appearance for the champion of Kirkwall" and the result is a completely different model that no one has seen before, it's either a major oversight or a bug. Otherwise, the option should be to "Use the new default appearance for the champion of Kirkwall".
It's not a bug. She was just designed differently.
The person in charge of modelling LadyHawke got a couple of things wrong. The hair obviously, but the jaw is very different and the eye color is also not correct. The only thing they got right is the nose.
The best thing you can do is create a default Hawke and model her to be more to your liking.
