The DxDiag reports several crashes referring to nvwgf2um.dll, which is from Nvidia, so it's related to your graphics card and/or graphics drivers.
According to your DxDiag, your graphics driver is from June 2015, but the Nvidia website has more recent drivers from 2016.

Maybe you should try that. If that fails, consider a clean driver installation. The website rules says I can't link to every site, but I have used Display Driver Uninstaller (DDU) in the past for this.
