dragon age inquisition connection to the dragon age server has been lost PC

IF this is single player and happens in combat, it is usually caused by a mage casting an upgraded fire mine spell.  I have had this issue on some runs and not others, seemingly sometimes that spell causes issues and other not.  Most people say that the character can be respec'd to not have that spell and it should fix the issue.

If it isn't about single player or happening during the game, then more info would be needed other than a single line of text to give context and possibly post a DxDiag file as well....
