Dragon Age Inqusition Jaws of Hakkon DLC question

As far as I'm aware this is not widely know problem, but I can't make the Firestarter trophy to pop.

I've cleared everything, I've reloaded the Old Temple and finished it close to 10 times.

I'm sure I've not missed anything, I've counted the fires and shards and got all of them. Just like all the guides and vides show.
I've got the one before door on the balcony, got the one behind locked door, waited for the boss to spawn his 3 shards. And nothing.
I have killed the boss trying different tactics - killing shards as they appear, leaving them to after the cutscene and mix of the above. Still no luck.

I've reinstalled the game and dlc, rushed thought using pre DLC save and nothing. Trophy still doesn't pop.
I've tried going outside the Temple and killing additional shards, I've tried leaving the fire behind locked door for after I've killed the boss... and so on.

At this point I'm out of ideas and have to assume that something is broken. 

I suspect that it might be the fire behind locked door in 2nd part of the temple. Even when I lockpick the door and gain access to the room, on the map there's still locked / padlock symbol. I've tried leaving this fire for the last, after killing the boss and returning just to lit it up - you can guess the result.

Any ideas?
