Dragon Age Keep still not updating decisions correctly

I am on my fourth playthrough and the Keep still hasn't updated the available World State options with some of the decisions I made in my previous three playthroughs. It seems like its always the same categories too. Here is a list of decisions I know I unlocked but the Tapestry says they're still locked:

Cassandra did not rebuild Seekers

Dorian stayed with the Inquisition

Sera did not kill Harmond and he joined the Inquisition

Fairbanks was not revealed as a noble

Inquisition forces deployed regularly

Sutherland's company was successful

Jecin and Celeste elpoed

Inquisitor contacted Charade

Inquisitor was a recruiting judge

Samson's Armor was not destroyed

Any thoughts on how to get around this?
