fade rift glitch

Having a problem with *mostly* a specific fade rift. Hinterlands (in the woods, act 1 PS4):

I'm getting literally THROWN out of combat when I leave my overhead combat screen. I'll be combing through these boards looking for similar problems. If anybody knows a way around this, please share.
