Regardless whether I kill her or decide to lie to Morrigan, she gives me the key, I go inside her hut, open both chests and get...a shortbow, and the other holds the unique robes for Morrigan. But there's no sign of the book.
I've tried reloading saves from before I entered the area and so on, and I'm fairly certain it isn't a mod issue as I don't have any mods that affect items, gifts or this questline.

I never had this issue myself so I did some research and found that from the Dragon Age wiki:
Flemeth's Grimoire may vanish from inventory if turned in more than once per Warden (i.e. save, hand it over, reload, hand it over again; reload again and it disappears
