I ran into a similar problem immediately after selecting the "master focus" perk. After building my focus to 300% and using my rift focus ability, the next time it charged, it stopped at 200%. I used the focus ability one more time and it no longer charges at all. Next, I made a trip back to Skyhold to find out that the Master Focus perk is unchecked/unacquired and my perk point lost. Bummer :/
It happens to me too. No solution?
