I work around this by realizing when I'm suffering from the bug and merely saving and loading at that time. Seems that the act of reloading the file alleviates the issue but this workaround isnt viable when facing a dragon or something when deaths are bound to occur. Feels bad when I go 3/4ths or half a battle with no more focus ability advantages.
If someone can recreate this bug then that'd be a load off of my mind since at the moment it could just be the way the game is performing for me...btw I play on PC if that makes a difference.

the focus gain, in order to get the 2nd and 3rd levels to fill up, you need to unlock them through inquisition perks, in the war room.
as far as i know it is not glitched. at least this particular thing hasn't been. but once you get the perks from the war room map, they work just fine.
I was able to unlock level 2 and level 3 focus in very short order once I got the vendor you by influence from (in skyhold)

Trust me, it's buggy. I posted about it in a thread I made a couple days ago and is linked in my above post. If I die during battle my focus meter, if all three bars are full, drops to one bar and will not refill until I take the steps I posted about.
Edit: I have a video showing it's buggy, just haven't uploaded it.

i have run into that too... but i don't think its buggy... at least to me it isn't.... I look at it like this....
well, technically you were knocked up.. errrr .... unconscious, I mean, yes? wouldn't that cause a loss of focus?
taking one to the knoggen, would definitely make me lose foucus, yes... at least partially?

No, my companions do not appear to lose their focus when they die and are revived. And you have to understand, once I die, it will not refill until I save and reload, the meter is stuck there forever at one bar. That is a bug that needs to be fixed.

In your case, yes, I would definitely say it is bugged/glitched.
