Have the same problem with this as well.
On my first playthrough, the operation shows up, and without doing anything, Leliana had already taken the operation even though she was already on another. So I thought that was normal and Leliana was just doing a special favor for Varric since I had Leliana do most of the Hard in Hightown operations. Nothing ever came out of that operation in the end.
On my second playthrough I was able to do the operation properly and when it was complete I would get the blank scroll.
