To get the fade scene Morrigan's Ritual must have been completed in Dragon Age: Origin (the Warden Commander must be alive and Kieran would be standing next to his mother in Skyhold prior to The Final Piece quest).

I'd started to think that my issue was because of the fact that I'd chosen Morrigan to drink from the Well Of Sorrows (which I did in my game) instead of my own character.
It seems I was wrong though.
I have played through Dragon Age Origins about 2 or 3 yrs ago. But when I finished playing, I'd uninstalled the game by this time.
I would like to know, if possible please.... is there some kind of connection between Origins and Inquisition as to how you end up meeting Morrigan's mother (Flemeth) in the fade?
Would I have had to keep Origins installed on my PC so the game knows to continue with the story in this way? (with the fade meeting) Or is there any other way?
Also, because I uninstalled Origins some time ago now, I don't remember exactly that far back, but I think I kept the warden commander alive when I played, though I don't really remember seeing Kieran (the little boy) standing next to his mother in Skyhold.

There's no direct connection between Dragon Age: Origin and Dragon Age Inquisition if you want to decide for yourself what decisions were made in the previous games (DAO and DA2) you have to use the Dragon Age Keep website. The area where you select your previous decisions is referred to as the Tapestry and one you've gotten everything the say you want it you can create a Wold State, which can then be uploaded into a new game.

Side Note: If you didn't upload a World State when starting the game, then the default state loaded and that would explain why you didn't get the Flemeth fade scene. In the default World State the Dark Ritual was not preformed and the Warden died killing the Archdemon .

Thank you for your extra helpful input.
As you have probably guessed, I didn't use the Dragon Age Keep website to setup and decide what happens in my play through of the dragon age stories.
It looks like there's a lot of stuff on there to look through... sort of makes me feel a bit confused over it. I'm not really very familiar with the website.
So yes, as you mentioned, looks like that's the reason for the story in my game being set to its default state.
As it stands, it looks like I'm not able to change the way the story is written in my play through at this rather late stage of where I'm at, considering.
I'm okay with it being this way. But I'm just glad I was able to find an answer to my confusion with DAI. So thanks a lot for that. 🙂
On another note, I also have a separate query to ask on the game regarding the Corypheus end battle which I might come back to ask about later.
But I don't want to feel like I'm taking up too much of one person's time, if there maybe anyone else around (anyone else who still uses the D.A. Forums here) who wouldn't mind to help and answer my question(s).

I'd recommend starting a new thread/topic for advice on the battle with Corypheus; the right title should draw in DAI battle-masters.
It's been so long since I've played DAI that I can only remember a lot of running. 🤔
