Same bug happening to me - female elf mage. It happens during cutscenes when I wear specific armor with "enchanter" shoulder upgrade (I don't know what is its english name, cause I have polish version of the game). Here's a screenshot from a cutscene :

Sorry, I couldn't find any better in my current saves.

You can see how misplaced the plate shoulder armor is. I couldn't get a good view on rest of the arm, but it's longer, the hands are deformed and barely (or not even) connected to the rest of the arm.
