Sometimes the companions/advisors just aren't there. There's nothing where they're supposed to be, like they vanished. Their icons are still on the mini map and everything, they're just not their. They come back eventually, but I can't figure out what causes it (maybe cutscenes?) or what brings them back (maybe area transtitions?).
It happens sadly. Saving and reloading helps for me. But all NPCs seem to experience this annoyingly.
Saving and reloading takes too long. It's easier to just leave the room and come back.
Its so strange, even if you turn a corner they dissapear as well. This should be fixed :/
Leaving/reloading doesn't always work so don't be surprised if it doesn't solve your problem. Cullen in particular appears to have issues.
I have the same issue - glad to hear it's not just me. Strange thing is, it seems like the NPC is still there, just invisible. It was most easily noticeable with Cullen in Haven. If I walked right to the mini map marker location where he's (supposed to be) standing, there is still something there blocking me. Like running into an invisible post.

Affected NPCs: Blackwall and Varric are pretty common. Due to the amount of NPCs I have to load around them. Some quest givers also suffer from this (most notably is Redcliffe village) but pop in eventually. Leiliana suffers from this as well if shes sitting at her desk usually. It happened for me more with Cullen when he is initially standing int he Courtyard before going to his room. Can happen with Cole and Sera too. Iron Bull is almost always missing initially when I load into the inn. Cassandra as well can go missing. Pretty much almost all are affected to some degree.
I didn't have it so bad in Haven. Cassandra and Leiliana were the biggest culprits in Haven. But in Skyhold seems like everyone does it.
Graphics settings wise I run with textures on Ultra everything else on low or medium. Changing texture quality to lower has never stopped this from happening for me though.
