I've got this problem too. Also on Vista 64. Anybody had any luck getting this fixed?

I posted this Solution in another Thread on Answers HQ but I'm reposting it here for anyone who hasn't seen/can't find the other thread
After Weeks of searching, trouble shooting, restores and driving my Multiplayer Buddy crazy with attempts to be heard/hear this is what I came up with:
-- Microphone: I turned my Input Level and Boost DOWN... yes I know this seems counterproductive but apparently if the mic boost is set too high, it overwhelms the sound channel
-- Speakers not hearing Player Voice: Somehow (I do not understand how since I did not mess with the files in question until I attempted to fix them) my ProfileOptions_profile file was changed from the settings it had a month prior.
Solution: I copied the ProfileOptions_profile file from another machine that my DAI was installed on and replaced the Corrupted?? one
The file can be found in Documents\Bioware\Dragon Age Inquisition\Save
Although PLEASE NOTE I used a text Editor Program (Notepad++) to edit the Resolution and monitor refresh values to match my PC so as not to freak out my monitor upon loading DAI
For 1920x1080 60Hz
Line 25: "GstRender.FullscreenRefreshRate 60.000000" (without Line 25: or quotes)
Line 32: "GstRender.ResolutionHeight 1080" (without Line 32: or quotes)
Line 34: "GstRender.ResolutionWidth 1920" (without Line 34: or quotes)
Or you could simply adjust these settings in the options on the Machine that does work to match a Res/Ref the malfunctioning Machine can handle before copying the "clean" ProfileOptions_Profile
