I'm experiencing the same problems for Cassandra. I tried like 5 times for the amulet of power during the quest "Champion of the Just". At first I thought it was because I skipped the "flag test" but after a couple of retry the chest only contains the seeker shield schematic and not the amulet of power for her. I did found the one for Cassandra in the Fallow Mire tough. I currently play the PC version of the game.
