I have the same problem.
NVIDIA GeForce GTX 750 Ti
8 GB RAM
Intel Core i5 CPU 2x 3,20 GHz (I'm aware that my CPU bottlenecks the Graphics Card)
Pre Patch I was able to run the game on a mix of medium and high settings with relatively stable fps.
Since the patch it stutters immensly (same settings), during everything, as described by the OP. Fighting pauses and goes into slow mo constantly, cutscenes stutter, even titlescreen and menus stutter.
(plus now it's shiny hair and no scars, smoothe textures time)
If I try the command line fix for the shiny hair problem, the stuttering gets worse (kinda expected).
Before the patch I had 107 hours of playtime with no problems and only 2 minor crashes. Now it's borderline unplayable.

Pre-Patch played the game on high/ultra settings, now it stutters all the time

Mantle or DX doesnt matter for me still sucks regardless....
went from "Hey I can play and it looks pretty sweet" to "WTF?!"

I rebuilt my damn rig JUST FOR THIS GAME! And Witcher 3. Not in my whole gaming life have I had a game tax my CPU so damn much. And it was fine before the patch. I expect crapdates from Microsoft, not from one of my favourite game companies on my favourite game.

Played for about 100 hours with no problem pre update and patch. I have tried virtually all of the fix solutions for the stuttering and freezing. I meet all the requirements to play (and again, had played a ton fine pre-patch). I have a GTX 750 TI and have the latest (new beta) driver recommended in the threads. I also competely reinstalled the game and the stuttering is actually now worse.
The game stuttering is so bad the game is now unplayable, as has clearly been reported here by many. It even halts and stutters on the black menu screens. Sometimes take as solid 10 seconds just to get it to recognize I have clicked on a setting button. I have a liquid cooled system with extra fans and am not overheating.
This is clearly a problem with the patch and I think I have done enough to be a good customer in order to fix it. It is in your hands now BIoware. Please get this resolved.
